How to Create Service ticket in C4C based on negative survey response in Qualtrics

Currently I am working on integration between Qualtrics and C4C system. The scenario is described in the title. I have created an oData servcie in C4C to create the service ticket but i am not sure how can i consume this service in Qualtrics. Would anybody be able to help me out in this.
