Multiple text fields on one multiple choice answer

Hi there,I am trying to figure out how to best format this particular question. The fourth option - "Graduated from a high school from..." -- I need two text boxes there - one for the State grad rate and one for the School grad rate. Even better would be labels for the text boxes. Better would be if I can make the text boxes required if they check that option. The best would be display logic that makes those two fields pop up in-page only when that answer is selected -- but I can totally live without that. (I know how to use Javascript, but I don't know how to write it)
