Create Custom links based on the responses people chose

We create a registration page where the business owner goes and selects the questions they want to ask their clients, which we have pre-populated. Once they have selected their questions, custom links are generated for the number of clients (links) they have requested. They then send out these links and each customer sees the questions the business owner selected. I think I accidentally erased some custom code in our survey because the links aren't being generated anymore. How do I get the links to generate?
