I need to program a questionnaire with the below layout, where a respondent can select one answer per row (as they are opposing statements)
Do I need to use custom programming? If yes, will a standard license that costs $30/month allow me to do that?

To do exactly what you are showing, you would need custom programming. I think you'd have to talk to a Qualtrics rep to confirm if that is available with your license.
That said, you might want to look at a Bipolar Matrix scale, which is CLOSE to what you are describing (has the same functionality with a different look and feel):
(Note that I changed the prompt so that labels are not required to understand the scale, and so that the reader is evaluating the statements from their own viewpoint (e.g. I wasn't 100% sure if you were asking them to evaluate whether "I like/don't like how I lead MY life" or "Some adults like/don't like how I lead MY life" or "I like/don't like how some adults are leading THEIR life" but it seemed most like the first one)

I contacted support and they pointed me to the community
I tried using Bi-polar, and it's my backup option as I need to use answer labels - they crowd to center, and I am worried it might confuse my respondents.

Gotcha. You could use labels like these:
Or even leverage a graphic for each of the "points" to force a set width between them.
Or maybe someone here will have some thoughts on how to apply JS for what you hope to accomplish. Good luck!
