Dollar Signs and Commas in Currency Question

I have seen some posts related to this, but cannot quite seem to find the right answer or get the Javascript correct. I am looking to add to a single line text question, a dollar sign either in the actual text box or to the side of it. Also I would like to use an automatic comma that appears for larger currency amounts. I am not a Javascript person, so if anybody has an exact screenshot of how this would look in the JS editor, it would be awesome. Thanks!
