I took a Home Depot survey a while ago and I'd like to create something similar. I'd like to do this to isolate the tons of responses we typically get like "no, n/a, I don't know, nothing" so we can focus on the substantive comments.

I tried 2 ways to recreate it:
Version 1 uses 2 questions: 1 text entry followed by 1 multiple choice. However, this adds a huge gap that I need to close with code. I don't know how to code.
Version 2 uses 1 question to display 2 multiple choice options with 1 text entry. The issue with this one is that I have to use 2 multiple choice options and I think this will cause confusion. Is it possible to hide the "my visit was" option?
Any ideas on how to achieve this?

If they start typing in the box, Yes is automatically selected for them, so it doesn't really require the extra click, but the Yes/No options are balanced and it doesn't seem "weird" that the text is there. Just a thought!
