I took a Home Depot survey a while ago and I'd like to create something similar. I'd like to do this to isolate the tons of responses we typically get like "no, n/a, I don't know, nothing" so we can focus on the substantive comments.

I tried 2 ways to recreate it:
Version 1 uses 2 questions: 1 text entry followed by 1 multiple choice. However, this adds a huge gap that I need to close with code. I don't know how to code.

Version 2 uses 1 question to display 2 multiple choice options with 1 text entry. The issue with this one is that I have to use 2 multiple choice options and I think this will cause confusion. Is it possible to hide the "my visit was" option?

Any ideas on how to achieve this?

I think you can simplify by using the second option, but making the question more of a Yes/No framing about WHETHER they would like to further characterize their visit. For example:
image.pngIf they start typing in the box, Yes is automatically selected for them, so it doesn't really require the extra click, but the Yes/No options are balanced and it doesn't seem "weird" that the text is there. Just a thought!
