How to repeat column headers in the bipolar matrix?

I am struggling to find a code or function to repeat by column headers in the bipolar matrix, as I have a big list in row option due to which there is a lot scrolling on page. Which is way I want to repeat the headers after 5 rows, to avoid bad responses.

    I am interested in finding code for a very similar formatting... a text vignette at top, with 13 bipolar matrix items below in alternating shading.
