I have used HTML to design the look and feel of the survey including the forms. Now i can’t see the feedback on Qualtrics. I can only see the question i didn’t use code for.

If you add html form fields to your question text, Qualtrics doesn’t know about them and won’t record them. To Qualtrics, it is just text.
