I’m trying to do a few things. Two screenshots are below.

I’d like to get rid of the numbers on the top of the line 1 - 5.

I’d like for each label to have a circle like the one that is under “Neither agree nor disagree.”

Finally, I’m trying to get the slider below to look like the second screenshot below. Someone said the second screenshot is a Qualtrics slider question. There must be a lot of JS and CSS which is not my strong suit. Or just get it as close as possible.

Someone took a picture of this with their phone. The line is actually straight.This is what they want it to look like or as close as I can get it. It looks like this is from a different online survey platform.
