I have another question - can I somehow limit the answer choices - so that for each row the participants will be able to only choose one of the four columns? (to prevent them from choosing multiple answers for the same statement)

I have used your java to create a similar survey. Yet It works with the first statement only. I am not sure how to move forward and include the following statements. What I'm missing is how to define that the second line is in fact a new statement requiring a new line?Here's what I've done (which is lacking something)
