Affiliation Question for a Social Network Questionnaire

Hi Qualtrics Community, 

I’m new to qualtrics and am working on putting together a social network analysis questionnaire where I am asking my respondents to  “Please list all the formal organizations you’re affiliated with and your role within them”

I am adapting this from a paper survey where I am asking them to manually write these down, but I want to have an online option.

Currently, I am using the text entry multiple lines option (pressing enter after each affiliation), but I would like to organize this reflect the separate entry format. I am also open to adding a multiple choice option for an individual’s role in an organization. 

Does anyone have any recommendations about how I would organize a question like this in Qualtrics?
