Ultimately I want to ask the same questions to 30 different organizations, but need to know which organization the data came from for geographic data analysis.

You can use query string parameters. Read the support pages on them. Pretty simple to implement.

Hello - thank you for sharing this option. It seems like there is a lot on query string parameters and the language is a bit above my comprehension level. :(
