I have a survey that mandated child abuse reporters will use to enter cases they hotline.  Our legal process is for these reports to be created and then sent to Department of Human Services (DHS) for further investigation.  We are asking our reporters to come back into the same survey a week later as a follow up and to report the outcome from DHS.

 

This past year I used the following methods to do this:

  1. one survey with initial report and then a second part for the follow up
  2. a STOP sign separating the two sections and instructions to close down the survey without continuing or submitting.
  3. an initial email with first responses sent in pdf to our child abuse prevention office inbox and to the reporter
  4. a second email a week later sent to the reporter with a link to the survey with instructions about completing the report 

The issue is that our reporters are either:

  1. submitting the initial report instead of just exiting the survey (then they can’t get back in to complete the survey)
  2.  just not coming back to complete it at all

I need to make this easier for the reporters as this is critical information that needs to be captured in our state.  

  • I could make the follow up survey completely separate and send an email with that survey link a week later, with the case reference number in the email subject line.
    • Issue- our child abuse prevention team wants each report to be together (the initial and the follow up) 

Are there any other ideas about what I could do? What I would like Qualtrics to allow is to PREVENT the reporter from submitting the second part of the survey until a week later, or linking an initial survey with a follow up survey and sending as one pdf to the child abuse prevention inbox.

 

Does anyone have any other ideas or suggestions about how to go about this?
