When selecting this question - it defaults to two columns. Is it possible to add more columns?

yes you can add columns in the side by side question.
- this is for adding column
- for adding statement you have “Statement” option above.
- for adding the answer click on ‘column option’» add column answer.

Yup. Just click on the ‘+’ icon in the left bar in options for column.
