Survey Question - How Do I create a Sub Question in a block?

I am trying to create a sub-question under the NPS question in one Block to ask the customer to "Please Explain their Score". I don't see any options to add a sub-question or I don't see any options to merge two blocks.

It's not a "sub-question", it's just a Text Entry question that you would create right under the NPS question to show up on the same page.
