I have used qualtrics a long time and never had this issue. I have created a survey, and I have named it and double checked the survey options to make sure it has the right name. However, when I click on the link for the survey or the preview, it has a name I never heard of. It is not even a name of any other survey I’ve developed - Michigan SBDC. I don’t want that on the survey.

It seems you may have inadvertently added the image in the 'Header' section. Please double-check, and if you require more information on headers, you can refer to the following link.
If the image is not in the header and the issue persists, it could be a bug or glitch. I recommend raising a support ticket with the Qualtrics support team for further assistance.
I see it - it was a preset option for a header that I didn’t choose - must have carried over from another department. I was able to get rid of it. Many thanks!
