Advanced Import Multiple Forms

Hello! Qualtrics newbie here! I am trying to import some (~700) questions using the advanced import method (https://www.qualtrics.com/support/survey-platform/survey-module/survey-tools/import-and-export-surveys/). There seems to be a [[Form]] option for form questions, but I need multiple (up to 5) form fields (or numerical entry boxes) instead of the default 0 for each of my 700 questions.
Is this possible, or do I need to split each of my form fields into separate Text Entry boxes w/ validation? Any help is appreciated!

Hi jmankewitz! It looks like you were able to reach out to our Support team about this one and they recommended setting up your form question like the following:
[[Question:TE:Form]]
Please fill in the following information:
[[Answers]]
First Name
Last Name
Email
