Creating tracking form for accidents
+2Has anyone created a form/process for identifying, tracking & mitigating accidents/ safety incidents?
+11I haven't done this specifically but I can imagine since at its core, it's a continuous improvement issue, that it would follow a DMAIC process: Define the problem, choose your Measurements, Analyze the data (including root cause), make Improvements, and then Control to make sure the improvements stay in place.
