Custom Formatting for side by side Matrix

Is there any way to validate the survey responses so that only one response gets selected from the entire row? Or would there be a better way to format this? !

I wouldn't use a side-by-side...just use a Likert Matrix with labels: !

    Thank you! Can you walk me through how added labels to your likert. When I do it the formatting is inconsistent with the numbers being out of line.
