How to create 2 text entry boxes in one choice in a multiple choice question

Hello Everyone,
I am creating a survey question that require 2 text enrty boxes in one choice in a multiple choice question. It seems Qualtrics only allow for 1 text entry box for each choice in the question. Here is an example:
In March 2020, what was the park you visited the most often with your child(ren)?

  1. Please enter: Name of the park [TEXT ENTRY BOX 1] Street Name [TEXT ENTRY BOX 2]

  2. I did not visit any park with my child(ren) in March

  3. The same park as before *

*so before this question we asked respondents to enter the name and street name of the park for another time period.
The picture shows that I can only add one text entry box.

Adding two text entry is not possible. You can break the question into two:
Q1. [Single choice] In March 2020, did you visited any park most often with your child(ren)?
Option 1. Yes, I.......
Option 2. I did not visit any park with my child(ren) in March
Option 3. The same park as before

If Yes is selected then show the next question
Q2. [Text entry form type question with two fields [Name and street name]

Yes, using JS we can have the desired question.

Hi rondev , You mention there is a java script or Jason command for having 2 text entries to a one of the choices in a multiple-choice question. Can you elaborate please? (need a separate text box for father/mother)
