I have ready (3) questions in 5-point Likert format. Question (1) for the respondent profile info. Question (2) will rate the importance of (40) performance skills (Very Low to Very High). Question (3) will rate the negative impact of (40) barriers on the performance.
I don't want to create (100+) individual questions. Instead, I want it to be simple for the audience to click the option against each skill/barrier.
Example:
Q. How important are the following skill areas for Logistics/Supply Chain managers?
Skill area None Very low Low Moderate High Very high
Collaboration 0 1 2 3 4 5
Also, I would need to have the survey in two languages options, English and Arabic.
Last, how the data analysis will take place? method? validity and reliability including correlation analysis.

If I am on the correct path of what you are asking, I would recommend using a matrix table for the 5 point likert. You may want to break up the skills into manageable categories so you have 4 matrix questions with 10 skills each. Matrix tables also provide for great simple charts in the dashboard.
