How to create a Letter Digit Substitution Test in a survey?

I am trying to create a letter digit substitution test in my survey, but cannot find any information on the question type I should be using for this. I need to create a row around 15 letters next to each other with a box under each letter to insert a numerical value. I need a minimum of 20 letter rows. If anyone has any tips, I would really appreciate it.
