How to?? Text entry next to a dropdown

I have this working with a Matrix table, but the problems are that:

  1. Super goal: to not have the text on the left at all ("individual 1 UTLN"), and only have the text entry and dropdown?

  2. I want to have a heading above both columns "UTLN" on the left and "Cadence" on the right. Alternatively/also, placeholder text in the dropdown of "Cadence".

  3. On a wide browser, the left column and right columns are left and right aligned, so they are REALLY far away from each other. How do we keep them right next to each other?

That's awesome, thank you!!!!!!! One last thing. The Side by Side question has 2 headers at the top of the left column and 1 at the top of the right column. How do I remove now one header? I only need it to say "UTLN" atop one column and "New Cadence" at the top of the other.

Actually, this totally works enough and I don't think it needs more effort. They can deal with a cap. Thank you again!!!!
