My survey has gone through multiple major revisions. In order help me interpret response data under each version I need some kind of indicator for each response to do this.

I don't know if there is a way to indicate which version of a survey that a response occurred under, but you could think about the way in which you revise your survey in order to make it clearer during data analysis/reporting. For example, instead of changing the response options, you can hide the response options that are no longer applicable and add new response options.
