I am trying to figure out how to best format this particular question.
The fourth option - "Graduated from a high school from..." -- I need two text boxes there - one for the State grad rate and one for the School grad rate.
Even better would be labels for the text boxes.
Better would be if I can make the text boxes required if they check that option.
The best would be display logic that makes those two fields pop up in-page only when that answer is selected -- but I can totally live without that.
(I know how to use Javascript, but I don't know how to write it)

I think you can create 2 separate options with text box, one for State grad rate and one for the School grad rate. Accordingly you will have to adjust the text for which the respondent is answering.
Or like you said, you can create 2 more text entry questions on same page and ask those if they select option 4. You can simply add the base logic for the 2 new text entry questions and select In page checkbox.
