Use the slider question type. You could change the grid lines to 5 to get the levels 0, 20, 40, 60, 80, 100. Do not snap to grid if you want them to answer 50.
@Sherly An easier way to do this without JS is to make that "Select button" question a Multiple Choice Question with Multiple Answer. Then, ensure that the display logic is in-page for the picture. If your select button is un-selected it should hide the picture. Alternatively, elaborate on when you want to hide the picture if you want it to be hidden so others can help with the JS code.
From my experience, I made a new contact list with the corrected email addresses and then I went to Distributions to "+ Compose Email" then sent it that way. To prevent needing to rewrite the message save the message. but unfortunately you will have to recreate the distribution and associated reminder emails.
@bansalpeeyush29 What I am trying to create are questions that for example look like this: 1. Please select areas we can improve on: - Sales Process * Set Expectations * Demo * Billing - Support * Response time * Adequate responses * POC reached * Ease - Product * Implementation * Features All these choices can be selected and the ones that are on the second level will only appear when the corresponding level above it is selected. Currently this is created with each of the first lv ones being separate individual questions without a question prompt and the second level ones are display logic. It's hard to analyze the first lv answers.
