jmborzick In the workflow, I split up the emails using conditions so both emails won't send. Email 1 is sent with a retake link:I can fill out the form 1 with the phone number but the form refreshes and does not end as it should in the survey flow. I do not receive the 2nd email with the 2nd retake link to fill in the room number even though I set the conditions as follows:https://cmu.ca1.qualtrics.com/jfe/form/SV_bl6No2bDFHFG3PM?Q_R=${e://Field/ResponseID}&Q_R_DEL=1&Approver=2Do I need to adjust my survey flow somehow?Thanks, Rosemarie

Each of the first two emails is appended with the respective approval and I can open each form.My issue is when I fill in the phone number or room number, the form doesn't end when either approver completes their part of the form, it just refreshes and it is not recognizing the "end of survey" in the branch logic.Then when both forms are completed, a final email should be emailed. Since I haven't gotten past the "end of survey" I don't know if the final email is sent.

I answered my own question, I realized I used a "form" question and not a multiple choice, all is good.
If I understand your question, you want to display a list of times but the user can only select one time from the list.When I set up student office hours, I list all the available times for each day as a multiple choice question type and use the List format allowing only one answer.If the time slot can only be used once, I set a quota for each time slot and then use display logic to not show the time if the quota has been met.I hope this helps!
In the past, after correcting/updating the email, I created another email distribution and sent it to only that one person from the contact list.

Good morning, Thank you for your recommendations, I added the "end of survey" after my fail block and I added an "Action" but I don't know if it is correct since I did not select a task to connect to API, Email, or distribute survey. Do I need to select a task? I never worked with actions before. ! I was able to get the form to work the way I wanted it to work using the appropriate retake link "http://cmu.ca1.qualtrics.com/jfe/form/SV_dgTP8x2afC74m8Z?Q_R=${e://Field/ResponseID}&Q_R_DEL=1" which I added to my End of Survey using the custom URL. Is there a way to avoid the slight lag of seeing the "end of survey" response and my reloaded survey? Your help is greatly appreciated!

My JS is attached .... I left the code as is and I changed the "#" to Q17 and neither work. ! Your example is exactly what I want!
! I the removed the script info from the CSS External and added the Flatpickr CSS and JavaScript to the header as you said (image attached) and no calendar appears. This seems to be very simple so I can't figure out what I am missing here.

I then changed '#' to Q17 which is the question number and still nothing happens so I changed it back to the original text. @bansalpeeyush29, @bstrahin - I got my computer equipment survey setup and it works the way I want it to except I would prefer not to have the user check the boxes again, is there a way to just use a bulleted list? I doubt it, but I thought I would ask as part of the learning process. I am running into issues on the back-end and I was hoping you could offer your help and expertise as to what I am doing wrong. Following is outline of my project and I attached a PDF with screen shots as well: Panel “CompAssetSept20” has ALL of the embedded fields below including first name, last name and email in my survey flow and I’m using email distribution. * Block 1 correctly pulls the equipment info that relates to the respective email from the panel * MC question has display logic on “is not empty” since everyone does not have 6 items * Block 2 carries forward the “selected choices from Block 1 and text area I setup the Contact List Trigger "Comp Asset Reply" to dump my feedback results but n
