If you have access to the new product ideas area, you can upvote my suggestion that they create a solution for this. :)
I found a way to get my survey to move along. I am using embedded data to create the logic based off of the total sum. I create a branch if the number is not within the approved range that goes to an error statement (question) and then has them go back to the original question to fix their numbers. We will see how this works.

We do collect emails whenever possible, but we do not sell direct and therefore at a good amount of our data is provided to us through our channel partners. A large portion of our historic data has company, address, and contact name but not necessarily an email. We could send an invitation via snail mail to take a survey including a QR code and/or an easy to remember URL, but will it be successful enough to offset the cost of mailing? Maybe.

You can edit messages in the Library found on the top right. Once there, go to the messages area and edit your messages from here. I think that is what you need. If not let me know.

You would use the paginated table setting for your comments. Under Data Source there should be a "additional columns" link. Add the Additional Columns that have your name source. You can add many of these if you like. You can also do a break out with the comments by another data source. Sometimes this is helpful in reviewing the comments.

@RachelTHREE - Let's say you have a survey of 5 questions. If you have logic in place that skips questions, your numbering is adjusted. For example: Q1, Q2 (answer from Q2 drives you to Q5) With dynamic numbering you would see 1, 2, 3 for your question numbers even though the third was actually Q5. The numbers are based on the order that the user views them.
