I'd like to identify on the subject reports who wrote what so that each open ended text entry comment has a name next to it. How do I do this?

You would use the paginated table setting for your comments.Under Data Source there should be a "additional columns" link. Add the Additional Columns that have your name source. You can add many of these if you like. You can also do a break out with the comments by another data source. Sometimes this is helpful in reviewing the comments.
