Just add labels (it's present on the left side of the question type selection box) and get 3 labels as required.
You will need 2 advance randomizer in survey flow, one for Q1 and next for Q2.Now, for Q1, set flags to 1 for all 15 questions (being shown in the same block in order) and randomly pick 5 of them. Then show the block with those 15 questions in order and use logic to be shown only if it's value is 1.Now for the second one, set all the 15 questions variables to 2 only if it's not 1. Example if Q1_1 (for first question) is not equal to 1 then set it 2 and so on for all 15 question flags. Then choose 5 from them randomly and again call the same block (duplicate the block in survey builder) and have logic to be shown only if it's value is 2.This will solve your requirement but will have 2 data columns for the same question. But then you can merge the final data in a 3rd block from both these scenarios.
The max you can use for grid lines is 20, so I would suggest to go with 20 grid lines and use snap to grid lines with 0 to 5000 max range. Let me know if this helps!
Why don't you use drill down question type and everything will be taken care by Qualtrics itself. In a nutshell it will directly filter all the required counties for that particular state which will show 2 drop downs in your case and drill the values to be shown specific to the state selected. Yup, you can do this but will need a series of code to run this using JS.Basically on the page submit of Q2 you will be moved to Q1 then (as both of them are of same type) you will punch the same choice as of Q2 in Q1 using .prop('checked',true) functionality and then click next on both Q1 & Q2. Meanwhile this JS is running, you will need to hide this from the person taking the link so hide the entire question.The above is just the method, hope this helps!
