How do I start building a consent form in a new survey?

Click Create project.

Select Survey, then click Get Started.

Name your project and click Create project.
How do I set up the consent question?

Click the text of the first question in your survey to add your instructions and consent statement (e.g., explaining that responses are anonymized and used for a research project).

Reduce the number of answer choices (e.g., down to two or three options).

Update the choice labels to clear statements like "Yes, I consent" and "No, I do not consent."
Using a Project Survey Template
Where can I find Qualtrics’ pre-built Informed Consent Form template?

From your homepage, click Create Project.

Search for "consent form" using the top-left search bar, OR select the Customers section and scroll down.

Click the Informed Consent Form project option.

Select Get Started, enter a name for your survey, and click Finish.
How does the Informed Consent Form template work? The template automatically generates two questions:

Instruction Question: Details on how to use the template. (Make sure to delete this question before finalizing and sending out your survey!)

Consent Question: Contains pre-filled text with editable sections tailored for your project's specific needs.
Ending a Session for Non-Consenting Respondents
How do I remove non-consenting respondents using Skip Logic?

Click on your consent question.

Select Skip Logic from the left-hand menu.

Set the logic to route respondents to End of Survey if "No, I do not consent" is selected.

Click Confirm.
How do I remove non-consenting respondents using Survey Flow?

Open your Survey Flow.

Add a Branch element with the condition set to trigger when "I do not consent" is selected.

Add an End of Survey block directly beneath the Branch element.
How can I customize the message shown to non-consenting respondents when they exit?

In your End of Survey block settings, click Customize.

Check the box to Override survey options.

Create a new message or select an existing message explaining why they have been removed.

Click Save and Apply to save your changes.

How do I verify that my consent logic is working correctly?

Click Preview at the top of your survey and test both consent options ("Yes" and "No") to ensure the routing and end-of-survey logic function as intended.
