We are creating a survey for a medical study, and would like to collect information about participants' smoking and drinking behaviors. We would like a feature that lets the participants input a number, then choose their unit of measurement (ie. 3 drinks a day, 2 drinks a week, 10 drinks a month, etc. I assume this is possible through Javascript, but I am no coder. I tried Googling a solution, but could not find anything. If anyone could help me out, I'd greatly appreciate it. Thanks!

I am not experienced with Javascript, but I believe you could accomplish something similar with a side-by-side question.
First column would be a text entry asking for the number of drinks. The second column would be a drop down with the choices of "a day" "a week" or "a month"
