Add Text Entry (multiple lines) and File Upload into a question

You can add a third question and use in-page display logic to show only one of the options.

you can add two question one would be text entry question and another would be file upload question. both will be displayed on same page and both will be non mandatory. thus user will be able to type in or upload and you can take care of validation using custom validation.
