Hi,
I need to add a question "Have you attended the training" and check box after.
If check box is clicked, new question will appear asking for date of training.
How can I achieve this?

Hi there, you can accomplish this by using Display Logic, maybe even In Page Display Logic. This will let you configure the date question to only display to respondents if a certain check box is clicked in a prior question. The below page has more information on it:
