Hi,
I need to add a question "Have you attended the training" and check box after.
If check box is clicked, new question will appear asking for date of training.
How can I achieve this?

+28Hi there, you can accomplish this by using Display Logic, maybe even In Page Display Logic. This will let you configure the date question to only display to respondents if a certain check box is clicked in a prior question. The below page has more information on it:
