Need to build a survey with Question Description with subquestions (10 or 20 questions with numbering) in qualtrics Example format : ! Thanks

For this, we need to use descriptive question as our first question and after that use text entry question.
Using CSS you can remove the space between the questions

Tried Not working with descriptive and text entry. Need to give choices( true or false) in the text entry . ! Please help
