How do I add a scale point (e.g., Not Applicable) to only select items in a matrix table?

I have a matrix table with 30 or so different statements with 3 scale points (i.e., Never, Sometimes, Often). For a few of the items, I need to add a 4th scale point (i.e., Not Applicable). Is that possible? I don't want the additional scale point to be available for the other items.

you won't be able to do it directly, but can accomplish something similar with a Side-by-Side question.
