Javascript Container Around Multiple Choice Question

Hey Qualtrics Community,
I need to put a container and additional labels around a side-by-side question, but I don't know how to do it in JavaScript and Qualtrics. The following image demonstrates what I am attempting to do.
Image one is a word version created by my team. Image two is what I have in Qualtrics, but it doesn't work for my team. Image three is a crude mock-up of what I am aiming for.

Hi, is the "familiarity" question the only actual part of the questoin they have to answer? In that case, I am thinking you don't so much need a Side by Side as a Matrix, with Category labels:

I put formatting on the Category titles (so they look ugly in the programming, but are more legible in the Preview):
image.pngThe grouping titles would not be labeled in the results, mind you, but could be added if needed into the variable labels.
