Javascript on form questions

I'm having trouble with spacing between labels on a form question. Both spacing between the next question but also justification/alignment of the form label and the answer box (and the size of the answer box. Changing box size to short had no impact on the preview of the question.
I was able to find the JS code to add the " $ " before the box but due to size it is appearing above it.
This is the "out of the box" display of the question. Pretty poor, so Qualtrics support sent me here for help. :(
PS. I'm not using the VanW survey template because I need to ask multiple VanW via randomization so I'm using the form question. Thanks!

Any code to just make this look somewhat professional is greatly appreciated!
