I am using the following HTML to insert a single checkbox in my question text:
Unfortunately, no data is being recorded from this. Can this be fixed using embedded data and/or custom code? I am completely lost, so any help is appreciated!

Yes onPageSubmit function, we can check if checkbox is checked and set an embedded data accordingly.
