Showing a calendar for the past 7 days and having participants fill out some information

Hi,
I am trying to make a question which shows a calendar for the past 7 days.
Each date will have two questions:
Number of drinks:
Hours drinking:

And participants fill out this for all the 7 dates listed. Can somebody help out?

You can use a text entry matrix and pipe in the last seven days as the statements: 

${date://OtherDate/DM/-7%20day}
through 
${date://OtherDate/DM/-1%20day}
. It would look like:
