Using CSS code to enable an cookies preference centre overlay to a survey

Hello all,
We are investigating different approaches to add cookies preference centre overlay to a survey. In essence, we must have an overlay added to the first question of the survey, that will force the respondent to accept/deny cookies. Have any of you built something similar?
