Is there a question set / block that exists in Qualtrics for a 360 or do I need to start from scratch. We are currently just testing the functionality, so plan to change he questions for our purposes, but am looking for a starting page.

Like a normal survey, you have to start questions from scratch. Most of your question will be rating scale. other than that you can include demographics or work culture related.
