Hi, I am new to Qualtrics and not sure how to build the below question. If anyone can help, it will be greatly appreciated.

      Amount Refused / Don't know
1 Brand   999999
2 Etc.   999999
3 Etc.   999999
4 Etc.   999999
998 Other (Specify)   999999

 What Question type should be used and how to create a D/K check box next to each numeric input.

I think you might need to elaborate a little on what are some of the information provided and how is it associated to the “question”.

1) What does “Amount” mean? Assuming numeric input, is there any association between the numeric input to brand and 999999?

2) What does “Refused/Don’t know” mean and how is it associated with 999999?

3) What does the numbering on the left column mean? Is it the number of statements or recode values?

(e.g. maybe it is for a respondent to indicate the expense spend (Amount) up to a threshold of 999999 for a service provided by a certain company, three stated companies and optional for another proposed company, with a checkbox for Refused/Don’t know when the service provided could meet your requirements.)

 

Based on the limited information, maybe you might want to look into Side-by-side question if you wish for respondents to answer various aspects (select and input values/text) for various statement within a single question.

It can only go up to 100 statements but you can consider having more Side by side questions if required.

Thank you very much for your feedback, and apologies for the limited info. Will keep in mind for next question. Your assumption is correct, Amount means Minimum 0, maximum 1 000 000 and D/K is just as you have indicated. Again thank you, mush appreciated.
