I would like to create a survey sent to recipients from a list that would also ask them to verify contact information and update it through the survey, if necessary. Each recipient would get individualized surveys according to their contact information on just this one question and then their yes/no response should prompt them to enter the correct information. But I’m not sure how to set that up. Or if it’s even possible.

One way to do this would be to use Piped Text from an Embedded Data field to display information from the Contact List to the respondent and ask if everything looks okay Yes/No. If no, they can be prompted to update their information in a Form Field question with Default Choices. Could look something like the below:
