Hi,
I would like to add another row of headers/groupings to a Side by side question. Attaching current and mockup images. Is there a way to do this? (using Java Script, maybe?)

You can have a view as shown below without JS. You need to just insert a Table from the Rich content editor. I have included the code for both tables you can insert in top headings ("Click to write Column 1") source and insert 4 answer columns in each. 
Table:
1st:
| Summer | |
| First Hour | Second Hour |
2nd :
| Winter | |
| First Hour | Second Hour |
