Audio file stops loading and error message

Question

I have been uploading audio files to enable Qualtrics to be accessible for a survey aimed at people with intellectual disabilities. I have been uploading the audio files and they are saved in the Qualtrics library.  However, one (184KB) has the error message ‘ This plug in is not supported.’  Can anyone advise why this error has only just occured?
