I’ve read through multiple posts and none solutions seem to do exactly what I need. 

GOAL: I have a matrix question that has statements carried forward from a previous question (vertically). Across we have a 5 point scale. We only need to carry through the statement options (vertical) that selected any of the following scale choices (once within the last week, once within last month, once within the last 3 months - across). 

WHAT I’VE TRIED: Recoding the scale points and using JavaScript (← novice, but I tried it). Carrying forward option, but you can only make one selection at a time. There isn’t an option to add an and/or connection for multiple statements to come through. Also tried piping through only responses that select each scale point on a single line, but this puts multiple statements on a single line, instead of assigning one to each line. 

If anyone has any solutions, I would greatly appreciate them.
