I am trying to build a survey to collect peer evaluation for a group project. The survey will consist of 5-8 questions. There can be 3-5 students in a team. The scale will be points from 1-5. Figure shows one way to collect this data. (No self evaluations).
The survey can be 3 questions. Question 1, student select their name. In question 2, student can be asked to select their team members. Third question should display a question similar to the figure above. They can either select the score or enter the value.
Is it possible to remove the student's name from the list of question 1 if they have already completed the survey.

Hi nsingh8,
To avoid multiple answers and asking the name of the respondent, you should use a contact list with personal survey link.
You will just have to compile a list of all students with their name and email before sending them the survey.
With this solution, you will also be able to send reminders to students who did not complete the survey after a few days and increase your response rate.
