Converting a Jotform survey/form into a Qualtrics form

Hi, I’m new to Qualtrics. I would Jotform for most of our more intricate forms, which has a pretty straight forward platform,  however, our university has moved to Qualtrics. I need to convert this Jotform into a Qualtrics form. I was able to tackle some of the features needed, but it’s been extremely difficult to navigate through Qualtrics to try to achieve the look of the original form as well as some of the conditional formatting and calculations. Any suggestions or help would be greatly appreciated!

    After reviewing your form, it seems quite straightforward to create a similar one in Qualtrics. You'll need to use the following types of questions to create this form. I'm providing links for you to understand them better:
