Don't use the first headers and use the
element in the second row of label (e.g. Flag for concern

Yes) as a label in the middle element. It will get auto-repeated. If you want to increase the space increase the
element to twice as shown below. In your case since there are either 3 or 1 elements you would not face the issue of not getting a middle value. To remove the top labels just add a space.
